* You must have your CSUF email linked to a Google Workspace account to access and complete these forms.

** If there is an agreement between CSUF and your institution (for example using assist.org), the student needs to complete the GForm. If there is no such agreement, the student needs to submit a syllabus, a copy of the transcript, complete the Gform, upload these documents using the GForm, and submit the official transcript to the TDA office (mail or eTranscript).
